01 The short version

What sets medical weight loss apart from dieting.

Bloodwork tells the truth.
Metabolic panels reveal insulin resistance, thyroid issues, and inflammation that stall weight loss.
GLP-1 medications are not magic, but they work.
Tirzepatide (Zepbound) and Semaglutide reduce appetite and improve insulin sensitivity – when prescribed right.
$199/month covers the program.
Medical visits, follow-ups, and nutrition coaching are included. Medication is priced at compounding pharmacy cost.
This is primary care, not a weight clinic chain.
Dr. Shemiranei treats the whole patient – not just the number on the scale.

07 About the practice

Weight loss medicine, practiced with accountability.

Dr. Saied Shemiranei with his wife, reflecting the family-oriented care at APMUC

You don’t find this connection these days in doctors’ offices.

The medical weight loss program at APMUC is built around one idea: your biology is not a character flaw. Dr. Saied Shemiranei, medical director and founder, uses bloodwork to find the actual barriers to weight loss before writing a single prescription – whether that’s insulin resistance, thyroid dysfunction, or something else entirely.

GLP-1 medications like Tirzepatide and Semaglutide have changed what’s possible for patients who’ve struggled despite real effort. We prescribe them through compounding pharmacies to keep costs manageable, and we adjust doses monthly based on what your body is doing, not on a manufacturer’s default schedule.

Our clinic is at 1400 E Robinson Street, Orlando, Florida 32803. We see patients Mon-Fri, 9am-5pm. We speak English, Spanish, Farsi, French, and Portuguese.

Languages: English, Spanish, Farsi, French, Portuguese.
Credentials: Family medicine and internal medicine, with additional training in addiction psychiatry (American Academy of Addiction Psychiatry) and pain management.

Dr. Shemiranei took the time to listen to my concerns, explained everything clearly, and made me feel comfortable throughout the visit. It’s clear that patient care is a top priority here.

08 Questions

Frequently asked questions.

What is a GLP-1 medication?

GLP-1 receptor agonists like Semaglutide and Tirzepatide reduce appetite, slow gastric emptying, and improve insulin sensitivity. They were originally developed for type 2 diabetes and have shown strong weight loss results in clinical trials.

How much does the medication cost?

GLP-1 medication is prescribed through a licensed compounding pharmacy and priced at compounding cost, which is substantially less than brand-name pricing. Your physician will provide current pricing at your consult.

Is $199/month really all-inclusive?

Yes – physician visits, body composition tracking, nutrition coaching, and follow-up are all covered in the $199/month program fee. Medication and any additional labs are separate.

Do I need to exercise?

Exercise improves outcomes, but it is not a prerequisite. Dr. Shemiranei will discuss realistic activity goals at your initial consult based on your current fitness level.

How much weight can I expect to lose?

Clinical trial data shows 15-20% body weight loss on Tirzepatide and 10-15% on Semaglutide over 12-18 months with consistent use. Individual results depend on starting weight, adherence, and other health factors.

Are there side effects?

Nausea, mild GI discomfort, and reduced appetite are the most common early side effects. Most patients find they improve within 2-4 weeks. We start at low doses and titrate slowly.

Can I do this if I have diabetes?

Yes, and GLP-1 medications are often first-line for type 2 diabetes. Dr. Shemiranei will review your current diabetes medications and adjust accordingly to avoid hypoglycemia.

Do you offer telehealth for follow-ups?

Yes. Monthly check-ins can be done by telehealth for patients who prefer it. In-person visits are required for initial intake and any lab draws.

Is this the same as Ozempic or Wegovy?

Semaglutide is the active ingredient in Ozempic and Wegovy. Tirzepatide is the active ingredient in Mounjaro and Zepbound. We prescribe compounded versions of these same molecules.
